My hope is that the videos I’ve created to help you understand my approach will do just that. Give you an idea of how I see self-work, counseling, therapy, or whatever the hell you wish to call it. Please watch and keep track of any questions you may want to share with me.

You can submit basic information and Shay will be notified of your interest. He will then reach out to you, and coordinate a time to discuss what is going on.
Things you should know as you ponder the idea of working with me
• I offer a free 30-40 consultation. Your choice: phone call, or virtual video format (i.e. zoom, Facetime, WebEx)
• I only work with clients at the age of 13 or higher.
• My sessions last 50-60 minutes. And always begin at top of hour.
• I conduct sessions at my home office and through video conferencing.
• You are welcome to text me between sessions… if something is really bugging you. This requires a unique brand of trust and respect.
• My self pay rate is $100 per session. Guests are welcome. Just notify me in advance.

If the consultation creates a feeling that both sides are cool, then we book our first session.
I’ll email you a link to the client portal. Complete the 5 documents below and submit back to me. (Ideally before the first session begins).

c) Payment Authorization Form (you will verbally confirm this info for safety reasons)
d) Client Insurance Form
e) Client History Form (PLEASE don’t feel pressured by this…we’ll get to it in session).
If using an approved insurance plan, please email or text me photos of:
Valid ID (driver’s license, state Id, passport)
- Both sides of active insurance card
- If you are a dependent (child, spouse, partner) on a policy, I will need the following info for the policy holder.
- Legal Name
- Date of Birth
- Address (associated with policy)
- Phone Number
These administrative hurdles can be a pain. I try to keep them simple, so that we don’t cause any undue stress. Do your best… and communicate!

What To Know About Scheduling
WEEKLY OFFICE SERVICE SCHEDULE
Virtual/phone or in person sessions are:
- Monday – Friday 11am – 7pm
Saturday | OFFSunday | OFF
NOTE: First Session begins at 11am
Last Session begins at 7pm @ Simple Jones Office Location
Format Options for Remote/Virtual Sessions:
- Phone Call (self paying clients only)
- Zoom (Zoom link will be sent within 12 hours of session)
- Face Time
- Skype
Scheduling & Cancelation Policy
ALL sessions are booked directly through me. Either by phone, text, or email. ALL changes and cancellations MUST be received by phone call within 24 hours of the session. Cancellation inside the 24-hour window will result in a $75 fee. I will consider this fee based on circumstances. I will call you 5 minutes after the scheduled beginning time. The session will be considered canceled 15 minutes after. This is a two-way street. I will credit you $75 if I cancel on you, within 24 hours. I would MUCH rather that we reschedule these sessions. It’s more important to ‘get the work in,’ so to speak.
I’m a nice guy… I’m not here to profit from unforeseen issues. So please respect my time (my form of surviving) and I’ll do the same for you. 🙂
Session Reminders:
Once your information from the client portal has been received, merged into your file and approved, you will be eligible to receive session reminders. These reminders can arrive via text message or email. Your choice.
